I am under 18. What necessary documents do I need to get pierced with parental consent?
We require the following documents to perform any piercing on a person under the age of 18. These documents must be current and valid. No expired ID's accepted unless accompanied by a Military ID. This applies to parent/legal guardian and minors.
- State issued Drivers License/Learners Permit
- State issued Identification Card
- Passport
- Military Identification
- Permanent Resident Card
- School Identification Card
We also require a Birth Certificate in order to pierce or perform any procedure on a minor. The birth certificate must be an original or certified copy. If the adult has been assigned legal guardianship or custody of the minor, you must provide the Original Court Issued Guardianship/Custody Paperwork in conjunction with the Birth Certificate and Identifications for both minor and guardian.

No, your sister, aunt, or cousin cannot sign for you or on behalf of a parent or legal guardian.
